Please complete the circulator affidavit on your petition sheet using one of the following methods:

Option 1 – Adobe Digital Signature

1. Open the PDF petition sheet.
2. Navigate to the circulator affidavit/signature section.
3. Use Adobe's Sign tool to apply your signature (draw, upload, or create a digital signature).
4. Save the completed PDF.
5. Email the signed PDF back as an attachment.

Option 2 – Hand Signature

1. Print the petition sheet.
2. Sign the circulator affidavit by hand in ink.
3. Scan or take a clear photo of the entire signed page.
4. Save as a PDF (preferred) or high-quality image.
5. Email the signed document back as an attachment.

Please ensure all information on the affidavit is completed before submitting. Return the signed document to the email address provided by the campaign.
